Gastrointestinal bleeding in patients with severe head injury, hypertensive intracerebral hemorrhage, and ruptured cerebral aneurysm.

چکیده
As a complication in patients with head injury or cerebral stroke, gastrointestinal bleeding is an important problem in neurosurgery. We reviewed 411 cases of hypertensive intracerebral hemorrhage, ruptured cerebral aneurysm and severe head injury in order to ascertain the pathologic condition of the gastrointestinal bleeding and to introduce an effective method of therapy. In 411 cases, gastrointestinal bleeding was observed in 19 cases (4. 6%). Among the cases of hypertensive intracerebral hemorrhage or ruptured cerebral aneurysm, the frequency was high in males. Among the cases of severe head injury, the frequency was high in children. Dogmatyl® had no prophylactic effect for gastrointestinal bleeding. Administration of steroids had no relationship to gastrointestinal bleeding. Of the 19 cases with the gastrointestinal bleeding, laparotomy was performed on 6 cases. Of the 19 cases, 10 cases died. From our review, laparotomy is considered to be the most satisfactory method of therapy. 35

Ruptured middle cerebral artery bifurcation aneurysm manifesting as caudate head hemorrhage extending into the ventricle: case report and literature review
Ruptured intracranial aneurysm usually manifests as subarachnoid hemorrhage (SAH) or SAH with hematoma. Aneurysms that only present with intracerebral hemorrhage or intraventricular hemorrhage are mostly confined to the distal artery. It is very rare that ruptured middle cerebral artery bifurcation aneurysms manifest signs of caudate head hemorrhage extending into the cerebral ventricle. One su...

Arteriographically Visualized Extravasation in Hypertensive Intracerebral Hemorrhage • Seven cases are reported in which extravasation of contrast medium from the lateral lenticulostriate artery was observed on cerebral angiography performed in the early stage of hypertensive intracerebral hemorrhage. We advance the theory that continuous bleeding from the ruptured artery with mechanical destru...

A 77-year-old female presented with a rare aneurysmal subarachnoid hemorrhage accompanied by a remote hypertensive intracerebral hemorrhage. With a past history of hypertension, she suddenly developed right hemiparesis followed by delayed loss of consciousness. Left carotid angiography demonstrated a left internal carotid-posterior communicating artery aneurysm. The intracerebral hematoma was l...
متن کاملMorbidity and mortality associated with intracranial aneurysms.
Subarachnoid hemorrhage (SAH) is bleeding into the subarachnoid space. It may result from a rupture of an intracranial aneurysm, bleeding from an arteriovenous malformation, hypertensive intracerebral hemorrhage with extension into the subarachnoid space, hemorrhage from an intracranial tumor, trauma, and hematologic disorders. If trauma is eliminated as a cause of SAH, more than 80% of SAHs ar...

Massive upper gastrointestinal bleeding is a known complication of advanced sepsis in trauma patients and carries a grave prognosis. However, to our knowledge, an important cause of massive gastrointestinal bleeding, arterio-visceral fistula, has not been discussed in the literature. The most common cause of this unique clinical entity is penetrating high velocity abdominal injuries, altho...
